New Contract Workers

Community Support for Families Program can provide a link between families who are looking for a worker to support a family member with a developmental disability and self-employed individuals.

The CSFF Program collects applications, conducts police checks and meets with potential contract workers.

To become a potential self-employed contract worker:

• Fill out the Personal Data Form

• Bring Completed Data forms to the Community Support for Families Program office at 15 Rattenbury Street, Clinton;

• The CSFF Program will contact you to schedule an appointment to review your information, provide the necessary forms to complete a Police and CAS check, and a Release Form in order to share your information with families who are looking for a worker.
